New Jersey State law requires every child born on or after January 1, 1997, and entering or attending Grade Six or a comparable age level special education program on or after September 1, 2008.

• Must receive one dose of Tdap (tetanus, diphtheria and acellular pertussis) given on or immediately after the 10th birthday and
• Must receive one dose of a Meningococcal-containing vaccine given on or immediately after the 11th birthday.

Enrollment for 6th Grade

• Documentation of these two immunizations and current Physical Exam records must be received by June of 5th Grade.
• Exceptions include if your child’s 11th birthday falls after June 1st.
  • In this case, you will have a 30 day grace period from your child's 11th birthday to get vaccinated.  
• Throughout the year, several reminders will be sent out with constant contact and email notification.
• Please have your child submit this proof of immunization record to the Health Office directly where it can be filed and recorded.

Failure to submit these will delay your child's start date and parent portal access.
