The Allergy and Asthma Action Plan forms must be completed and submitted annually. The Action Plan should cover the entire school year. 

Medication must be in the original or perscription conatiner and not be expired. The medication sheet

Follow the Sports Forms and Sports Forms due date link to complete the required forms for athletics. The Parent Concent form must be completed for each sport your child would like to participate in. In addition to the medical forms and concent form, you and you student MUST both sign off and submit the acknowledgement forms after reviewing the  – Sudden Cardiac Death in Young Athletes brochure, Opiod Use and Misuse fact sheet, Concussion Fact Sheet.
